Required Information

To enable completion of your assessment supports, you must provide:

• Allergen declarations for every essential oil used, obtained from your supplier.
• Where essential oil blends are used, the percentage of each individual essential oil contained within the blend.
• All usage rates must remain within the limits permitted by the assessment addendum.
